A hot lunch. A grab rail for your bathroom. Completing an application for an apartment.
These simple things can make a significant difference in your life. In fact, they can make all the difference in helping you continue to live independently.
The Care Management Program works with you to plan your care. Your Care Manager will help you better understand the array of services that are available right in your own community. Specially trained in geriatrics, your Care Manager also has extensive knowledge of resources, matching you with those that can best suit your needs. Your Care Manager can also arrange for in-home support, such as: nutritious meals, gentle assistance with your personal care, skilled counseling, help with house and yard work, transportation to doctors’ visits, someone to pick up a few items at the store or the reassurance of a personal emergency response system.
Your Care Manager will ensure that you have as little or as much support necessary to help you live safely at home. But we don’t stop there. Your Care Manager will continue to work with you to guide your care and respond to any changes to your circumstances. The ongoing service has one goal: to be your bridge to independence.
Your entry into the Care Management Program begins with a visit from a Delaware County Office of Services for the Aging (COSA) staff member, who assesses your care needs and eligibility. Your SCS Care Manager will then look forward to working with you.
